Introduction:

*Do no harm to patient*

This is one of the principle in practice of medicine, and infection prevention plays significant role.

We at Lilavati Hospital and Research Centre came up with an innovative idea of creating awareness by learning with fun in various aspect of infection control.

The whole idea was not only infection control, rather its infection "In Control".

To know more:

1. Don’t Wait Vaccinate:

  • Vaccines have reduced morbidity and mortality of many diseases.
  • Intensified Mission Indradhanush 4.0 has been planned to reach out to unvaccinated and partially vaccinated children and pregnant women all over India.
  • Children and adolescents - Typhoid conjugate vaccine, single dose.
  • HPV vaccine - adolescent girls 9- 18 years which prevents invasive cervical cancer.
  • Need to spread awareness of adult immunization
  • Pneumococcal vaccines in immucompromised and patients above 65 years of age which prevent and decrease invasive pneumococcal disease related mortality.
  • In women MMR and varicella vaccination has a major role before pregnancy.
  • Rabies pre and post exposure prophylaxis along with use of monoclonal antibodies whenever indicated
  • Need of annual vaccination with influenza vaccine, 2 weeks before flu season in children, healthcare workers, immunocomprimised and elderly population.
  • New herpes zoster vaccine for elderly population to decrease incidence and severity of herpes zoster illness.
  • Hepatitis B vaccination for all health care workers and protective hepatitis b antibody titres.
  • Over the years, vaccines have prevented countless cases of disease and saved millions of lives. Vaccination is important part of family and public health.

2. A tale of antibiotic

  • Infections due to drug resistance are the leading cause of death around the world.
  • There are no new higher antibiotics in pipeline, and we can’t rely on the current in-use higher antibiotics against the pan-drug resistant bugs. Therefore, the only option left is preserving the current antibiotics in use.
  • Preserving indicates- Appropriate use, judicious use, No misuse, no abuse, no over use of antibiotics.
  • We at Lilavati Hospital and Research Centre have a stringent antibiotic policy as well as surveillance protocols to prevent drug resistance and pledge towards Antimicrobial Stewardship Programme.

3. Hand Hygiene Day

  • Hand hygiene and Isolation precautions
  • Most common mode of transmission of pathogen is via our Hands.
  • There is substantial evidence of relationship between hand hygiene and health care associated infections, which says that hand hygiene reduces the risk of infection.
  • Alcohol based hand rub / hand hygiene with soap and water should be followed with proper steps whenever there is opportunity amongst the five moments as recommended by WHO.
  • Isolation precautions create a barrier between patient and microorganisms and thereby, prevent cross- infections.
  • We at Lilavati Hospital and Research centre are proactively practicing standard precautions and transmission based precaution as and when needed for patient protection.
  • Transmission based precautions are practiced in the form of –Airborne isolation precaution, Droplet isolation precaution, Contact isolation precaution and Protective isolation precaution; as and when needed.

4. Bio medical waste management

Biomedical Waste(BMW) Management and its importance

  • BMW is generated by healthcare facilities and can be hazardous and infectious.
  • Improper handling can lead to health hazards, so collection, segregation, transportation, treatment, and disposal of BMW are important steps in its management.
  • “success of BMW management depends on segregation at the point of generation”
  • The color coding of bins, the use of technologies such as incineration and autoclaving, and attention to environmental impacts are also highly crucial.
  • BMW management aims to reduce waste volume and ensure proper disposal, hence the basic principle of the management of BMW is Reduce, Reuse, and Recycle-the 3Rs
  • All those involved should strive to make the environment safer.
  • We at Lilavati Hospital and Research centre are committed for proper handling, Segregation, treatment, safe disposal and transfer to the final waste treatment plant.

COVID Antigen Testing

What is the rapid antigen detection test for Covid-19?

  • It is a test on swabbed nasal samples that detects antigens (foreign substances that induce an immune response in the body) that are found on or within the SARS-CoV-2 virus. It is a point-of-care test, performed outside the conventional laboratory setting, and is used to quickly obtain a diagnostic result.

  • In India, the ICMR has allowed the use of antigen detection kits developed by the South Korean company S D Biosensor. The kit, commercially called Standard Q COVID-19 Ag detection kit, comes with a with an inbuilt COVID antigen test device, viral extraction tube with viral lysis buffer and sterile swab for sample collection.

Principle: Rapid Chromatography

Advantages:

  • On the positive side, antigen tests can be run quickly and produce results within one hour

  • Antigen tests are important for the overall COVID-19 response, because they’re simple and cheap to make

  • This type of test has the potential to scale to millions of tests per day.

Where will the test be used?

As of now, the kit will be used in containment zones or hotspots and healthcare settings.

  • In containment zones, the test can be conducted on all symptomatic influenza-like illnesses. Asymptomatic direct and high-risk contacts with co-morbidities (lung disease, heart disease, liver disease, kidney disease, diabetes, neurological disorders and blood disorders) of a confirmed case are to be tested once between day 5 and day 10 of coming into contact.

  • In healthcare settings, it can be used in three categories.

First, in all persons presenting influenza-like symptoms in a healthcare setting and suspected of having Covid-19 infection;

Second, in asymptomatic patients who are hospitalised or seeking hospitalisation, in the following high-risk groups — those chemotherapy, immunosuppressed patients including those who are HIV positive, patients diagnosed with malignant disease, transplant patients, elderly patients (over age 65) with comorbidities and

Third, in asymptomatic patients undergoing aerosol-generating surgical/non-surgical interventions such as elective/emergency surgical

Procedures like neurosurgery, ENT surgery, dental procedures, and non-surgical interventions like bronchoscopy and dialysis.

Is the test a confirmatory one for diagnosis of Covid-19?

According to the ICMR guidelines, if the test shows a positive result, it should be considered as true positive, and does not need reconfirmation. However, those who test negative in the rapid antigen test should then be tested by RT-PCR to rule out infection.

Lilavati Hospital Blood Bank

Majority of blood banks in city are cheating patients

By Jyoti Shelar, Mumbai Mirror | Mar 31, 2015, 12.00 AM IST

An FDA probe against 19 blood banks, including those attached to leading hospitals, has revealed that just three of them - Hinduja, Lilavati, and BD Petit Parsee Hospital - were charging patients correctly.

The Food and Drug Administration (FDA) probe into 19 blood banks in the city has found that majority of them have been charging more than the rates prescribed by the state government.

According to the FDA's joint commissioner, Sanjay Patil, only three blood banks- attached to Hinduja Hospital, Lilavati Hospital and BD Petit Parsee General Hospital - were found to be accurately charging patients. "The remaining 16 blood banks were over charging anywhere between Rs. 50 and Rs. 500," Patil said.

A government resolution dated June 2014 has set Rs. 1,450 as the price for one unit of whole blood, Rs. 1,450 for packed red cells, Rs. 400 for fresh frozen plasma, Rs. 400 for one unit of platelets, and Rs. 250 for cryo precipitate.

In November last year, the FDA officials inspected 309 blood banks across the state, of which 72 were found to have discrepancies in charging process. Out of the 72 blood banks, 19 were from Mumbai.

"Detailed inquiry was conducted in all these 72 blood banks including 19 in Mumbai, and merely three were given clean chit," Patil said, adding that in Mumbai, blood banks of leading hospitals such as Jaslok, Breach Candy, Cumballa Hill, the Bombay Hospital, Asian Heart Institute, Prince Aly Khan, Kokilaben Dhirubhai Ambani, SevenHills Hospital, Raheja and Fortis were found to be overcharging patients. Among the independent blood banks, the Borivali Blood Bank, Suburban Hightech (Malad), Manas Serological (Jogeshwari), West Coast (Mahim), SR Mehta Cardiac Instate (Sion) and Ashirwad (Dadar) were found to be over charging patients.

While the FDA had already issued show cause notices to the blood banks, the names have also been sent to the State Blood Transfusion Council (SBTC) for action. "After we issued notices, these blood banks have corrected their rates.

However, action has to be taken as they were found over charging during the inquiry," Patil said, adding that there is no act or clause under which FDA can take action. "Since SBTC has the powers, they will have to initiate action," he said.

This Board caters to the much desired needs of the consumers and has set standards for progress of the health industry. NABH is a member of the International Society for Quality in Health Care [ISQua] and thus NABH standard is recognized globally.

The board is set up to establish and operate accreditation programme for healthcare organizations. NABH have designed an exhaustive healthcare standard for hospitals and healthcare providers. This standard consists of stringent 600 plus objective elements for the hospital to achieve in order to get the NABH accreditation. These standards are divided between patient centered standards and organization centered standards. NABH accreditation helps planners to promote, implement, monitor and evaluate robust practice in order to ensure that it occupies a central place in the development of the health care system. Thus the objective of NABH accreditation is on continuous improvement in the organizational and clinical performance of health services, not just the achievement of a certificate or award or merely assuring compliance with minimum acceptable standards.

How NABH accreditation helps patients:
In an NABH accredited hospital, there is a strong focus on the following:

  • Patient rights and benefits
  • Patient safety
  • Control and prevention of infections
  • Practicing good patient-care protocols e.g. special care for vulnerable groups, critically ill patients
  • Better and controlled clinical outcome.

So, if a hospital is NABH accredited, the patient can be rest assured that the hospital follows stringent standards as laid down by the accreditation body for providing best in patient care comparable to any international hospital of repute.
